This is to inform you that I am a regular student of class vi of your school. I could not attend your school from 10-09-17 to 12-09-17 on account of my fever. A medical certificate is enclosed herewith for your kind consideration.

I, therefore, pray and hope that you would be kind enough to grant me leave of absence for those days only and oblige thereby.
