Dear Sir,

I hope you are well. I wanted to let you know that I came to school today, but I'm not feeling well. I have caught a cold and have a fever. Because of this, I can't stay in the classroom anymore.

Could I please ask for your permission to leave school early? I would be very grateful. I promise to make up for the missed periods later.
